LIABILITY: In consideration of the acceptance of participating in THE TEMPE BEACH CONCOURS, by execution of this entry form, I hereby release and discharge Modded Kulture LLC and anyone else connected with the management and promotion 

of this event, including all sponsors and vendors, from any and all known and unknown damage, injuries, judgments and/or claims from any cause whatsoever that may be suffered by any member to myself, my person or property. 

Further, I the applicant, expressly agree to indemnify all of the foregoing entities, firms, persons, and bodies of and from any and all liability occasioned or resulting with the applicant and under the direction or control of the 

applicant.

MEDIA RELEASE: I hereby consent to release for possible publication my name and/or photo(s) and /or video images taken by any authorized Modded Kulture LLC, employee or staff of this event, or any media representative for news 

and/or publicity purposes. This includes social media sites (Instagram, Facebook, Twitter, Snapchat, etc.) and/or Modded Kulture LLC, (newsletters, Blogs, website, advertisement, etc.). I also agree to allow the publication or 

broadcast use of my name in connection with any photo(s) and/or video images taken, and I understand that I will not receive any payment or gift of any kind for my voluntary participation or future use of any photo(s) and/or 

images of me or my property or entry. I understand that photos and /or videos for the media and /or World Wide Web may be used in publications and /or Web sites outside of Modded Kulture LLC control.

PROMOTIONAL EMAIL: By providing my email I agree to receive email or physical mail from Modded Kulture LLC and our sponsors or vendors.
